Transition Presentation with Cheryl Chan

“22 at 20: A Nontraditional Transition Story” has been presented to schools, the DESE, at conferences and for adult organizations across Massachusetts since 2012.  It is the unprecedented story of collaboration between a school district, family, 2 adult service providers, DDS and UMASS over the course of 1 year, to transition a young man from a hospital-based residence for children with autism & severe behavioral challenges, into a living model that no one thought would ever work for him.  Yet, 3 years later, the progress has been astonishing; exceeding all expectations.  We present the history, timeline, components of the collaboration, each participant’s roles, and advice on how this transition model can work for others in part or in whole.  It is not just about people with severe disabilities; the takeaways have been named “best practices” by DESE.  Presenters are Cheryl Chan, mother of the young adult in the story, and Philip Campbell, the Director of Special Education during the time of the transition.  For more information please visit
